Chapter 6 Which department are you a doctor in?
Fang Zhiyan stepped closer and looked again to make sure.
The girl was unconscious, and under the man’s pressure the severed jugular vein was indeed no longer bleeding.
But she was not breathing either.
“Sir.”
Fang Zhiyan called out and crouched down. “Your pressure point is wrong.”
“You’re hurting her like this.”
At Fang Zhiyan’s voice, the man frowned. “Who are you? I’m a doctor from the People’s Hospital. Are you saying my first aid is wrong?”
“I’m a doctor too,” Fang Zhiyan said.
But before he could finish, the man grew impatient.
“I’ve been seeing patients at the People’s Hospital for more than ten years. And you’re teaching me how to do my job?”
“I remember anatomy. How could I not know where the jugular vein is?”
Fang Zhiyan paused and looked once more at the injured girl.
Because her windpipe was being compressed, her face was beginning to take on a faint purplish tint.
The situation was extremely critical, and Fang Zhiyan had no time for an argument. He acted at once, swiftly shifting the man’s hand to a different pressure point.
The man flew into a rage and was just about to speak when the girl beneath him suddenly drew in a breath.
The choking in her face eased somewhat as well.
“If she were a healthy adult, the spot you pressed would not have been a problem.”
“But this girl is young and slight, so when you pressed her jugular vein, you also pressed on her trachea and prevented her from breathing.”
Fang Zhiyan explained quickly, afraid of provoking unnecessary misunderstanding.
Hearing this, the man looked at Fang Zhiyan for a long moment but said nothing more.
The crowd around them nodded in agreement.
In those days, not many onlookers had phones capable of recording video, so they could only watch and could not share what they saw.
Fang Zhiyan lowered his head and checked the time.
The traditional hospital was very close by, and by rights the ambulance should have arrived quickly.
But it was the morning rush hour, and ambulances were not plentiful in this era.
So it was very likely to be delayed on the road.
Fang Zhiyan looked once more at the injured girl before him.
Her body was covered in shards of glass, and the veins in her arm were visibly distended.
In general, venous distension is caused by increased pressure within the chest cavity, and at this moment the girl’s rib cage was clearly making abnormal movements.
That meant she was showing a rare, fatal sign.
Traumatic pneumothorax.
At that realization, Fang Zhiyan himself broke out in a cold sweat.
Traumatic pneumothorax is generally divided into three types: open pneumothorax, closed pneumothorax, and tension pneumothorax.
The girl’s breath sounds were weak, and she had just been struck by shattered glass, so the first type, open pneumothorax, was highly likely.
This rare and fatal condition could not wait even a moment.
If the underlying cause could not be found and relieved in time, she would undoubtedly die.
At this point, waiting for the ambulance was no longer realistic.
A person was right in front of him; he could not fail to save her.
Fang Zhiyan stood up quickly. “Who has a knife? Is there any disinfecting alcohol?”
“This girl has traumatic pneumothorax and must be treated immediately.”
“What?”
The man holding her jugular vein was shocked as well.
He was not a surgeon, but he knew how rare and dangerous traumatic pneumothorax was.
“There’s a pharmacy next door!”
The breakfast shop owner shouted and pointed in the other direction.
Fang Zhiyan did not hesitate for a second and rushed straight to the pharmacy.
“I need high-proof alcohol, a small knife, and tape!”
The pharmacy owner was also there watching the commotion.
When he saw Fang Zhiyan, he wasted no time and quickly prepared everything Fang Zhiyan needed.
His speed surprised Fang Zhiyan a little.
But now saving a life was like fighting a fire, and he had no time to say more. He thanked the owner and hurried back.
The girl was still lying on the ground, but her condition was clearly worsening.
Fang Zhiyan could not wait any longer. He gently lifted the girl’s clothing until he could faintly see the lace trim, then quickly stopped.
He put on rubber gloves and used the high-proof alcohol to disinfect the gloves, the plastic tube, the knife, and the surface of the skin.
Such swift and practiced movements made the middle-aged man nearby twitch at the eyelids.
He wanted to say something, but every step Fang Zhiyan took ran beyond his expectations.
He himself was an old doctor who had been seeing patients for more than ten years, yet he could not keep up with this young man’s pace at all.
Fang Zhiyan took a deep breath, then used the knife to cut the plastic tube and inserted it into a saline bottle he had borrowed from nearby.
Next, holding the knife, he searched the girl’s chest for the exact site of the open pneumothorax.
After confirming the location, Fang Zhiyan steadied his hand and, without hesitation, drew the blade down along the grain of the skin.
Exclamations rose from all around.
Some people even shut their eyes at once, unable to bear watching.
Only the middle-aged man across from him was once again stunned by Fang Zhiyan’s skill.
The cut was exact, no more than necessary, following the skin’s subtle lines.
If it healed in the future, the scar would be negligible.
The blade had fallen with astonishing precision.
The middle-aged man looked deeply at Fang Zhiyan before him and began to feel curious about him.
After cutting open the skin of the chest, Fang Zhiyan quickly inserted the plastic tube.
In just a moment, bubbles began gurgling up in the saline bottle.
At the same time, the girl gasped for air, and her condition improved rapidly.
Only now did the middle-aged man confirm that Fang Zhiyan had truly saved the young girl’s life.
Looking at the wound Fang Zhiyan had made, then at the homemade one-way valve Fang Zhiyan had fashioned, he was struck with overwhelming admiration.
“Little brother, you’re amazing. You saved her,” the middle-aged man said with emotion.
“Which hospital do you work at? I’ve never seen you before.”
Hearing this, Fang Zhiyan looked up and grinned at him.
“I’m a doctor in the emergency department of the traditional hospital.”
“The traditional hospital?” the man said in surprise, not expecting such outstanding talent there.
The surrounding onlookers were equally amazed.
“My goodness, he’s actually a doctor from the traditional hospital. Young, handsome, and so skilled.”
“Next time I get sick, I’m going to the traditional hospital for a look.”
“I never expected the emergency care there to be this strong.”
Amid the crowd’s chatter, the sound of an ambulance came from afar.
When the vehicle stopped, a doctor hurriedly jumped down. “Where is the patient? Where is the patient?”
Together, they lifted the girl onto the ambulance.
Fang Zhiyan simply got on as well.
He was from the emergency department too, and after being delayed for so long, it would be faster to ride in the ambulance.
With the patient aboard, the ambulance wailed away.
Back in the emergency department of the traditional hospital, the intern doctor Zhu Zixiao was busy lodging complaints.
“Teacher Cao, this Fang Zhiyan was late on his very first day at work.”
“As a doctor, how can he have any sense of time?”
“You should be careful with him. Don’t let him drag you down later.”
